Subject: Consent / Waiver Form for 2026 Summer
Below is a consent form giving your son/daughter permission to participate in Electric Works LLC summer baseball program. This consent form allows your child to practice and play games. lt is also a waiver of injury. Guardians understand that Electric Works LLC is not liable for any injuries that may occur while participating in this program. lt is understood that Electric Works LLC cannot be held liable for any injuries that occur while participating in the Electric Works baseball program.
l/We are not aware of any injury illness or other health related issues that would restrict or limit my players’ ability to play competitive sports.
l/We agree to Assume all risks and expenses due to an injury that may occur as a result of my child’s involvement in competitive sports’ practices, games and/or travel to and from said activities.
l/We agree to not hold Electric Works LLC or anyone acting on its behalf either as a coach, assistant or administrator harmless in the event of an injury to my child while participating under the supervision of the above.
